Program eligibility

  • Completion of ACGME-accredited Cardiovascular Disease Fellowship
  • Board eligible in Cardiovascular Disease at start of advanced fellowship training

Apply through ERAS

Applicants may apply to the Aurora Heart Failure & Transplant Fellowship program via the Electronic Residency Application System (ERAS). ERAS begins accepting applications in July. Our program excepts ERAS applications through December 31st of each academic year.

A completed application must include the following documents:

  • ERAS common application form.
  • Photo.
  • Personal statement.
  • Curriculum vitae (CV).
  • USMLE/COMLEX scores (1,2,3 steps).
  • Copy of ECFMG certificate (if applicable).
  • Three letters of recommendation, one of which must be from your current or prior cardiology program director.
  • If you’re a non-U.S. citizen, you’ll be required to provide a copy of your resident status if you are invited for an interview. Aurora Health Care does not sponsor H-category visas.
